Our support workers offer various support workers, including healthcare services to assist our participants with their mental, health and physical conditions. Our support workers are compassionate members of our team and understand every participant’s needs in a unique one-on-one standing.
Our support workers are highly trained to deliver the support services needed, to care, nurture and empower our participants in a respectful and compassionate manner. Their roles include but not limited to, meal preparations, emotional support, personal care, personal support and engagement in daily living activities.
Our support coordinators also play a vital role in supporting our participants offering a mix of supports to increase the capacity to maintain relationships and manage service delivery tasks, centred around our participants being able to live more independently and have a better representation within their communities.
Our support coordinators work through the complexities of our participants plan and we provide timely sessions to interpret the particulars of the plan. This ensures our participants are made well aware of the support services being provided to them.
Our participants are empowered with an individually crafted support pathway plan, each of our support coordinators have extensive networks, across a wide range of stakeholder supports endeavouring quality and assurance when caring for our participants.
Our Support workers as per our level 2 coordination services we support our participants in collaboration with other stakeholder supports both Government and private agencies. This can also include family members, guardians and other persons of importance in ensuring the best outcome in the services we provide.
Our support coordinators continuously monitor the supports to ensure the participant is effectively receiving the support required. Our support coordinators continuously monitor the progress of the supports provided to ensure the participants are benefiting to their full capacity.
Level 3 Coordination
Our level 3 support coordination works with our more complex participants. Our specialist support coordinators assist our participants to overcome complex barriers in their support environment. Our support coordinators are appropriately qualified and experienced to deliver these requirements.
Our specialised support coordinators identify and understand these barriers impacting the participants ability to access, use and maintain their supports. Find solutions to address these barriers, establish a process to overcome these barriers and identify any additional supports to be in place immediately.
